When Microsoft Entra external MFA is not working, start by locating the failed handoff: policy evaluation, method selection, redirect to the provider, provider authentication, or token validation on the return to Entra. Each stage has different evidence. Disabling Conditional Access or rebuilding the provider configuration before you know the stage usually destroys the trail you need.

The short answer is this: confirm that the provider application has admin consent, the external method is enabled for the user, the user is registered, and the Conditional Access policy uses Require multifactor authentication. External MFA does not currently satisfy an authentication strength. Then correlate the Entra sign-in record with the provider's record by time and request identifiers.

Grab a coffee and keep one failed test sign-in open. This guide covers Microsoft's generally available External MFA feature, formerly called external authentication methods. It does not cover AD FS MFA adapters, NPS extensions, or the design of a third-party provider itself. Microsoft announced External MFA as generally available in March 2026, with Entra remaining the identity control plane for policy and access decisions. Microsoft's Entra release notes define that supported model.

Microsoft Entra external MFA not working: classify the symptom

  • External MFA is missing from the method picker: inspect method scope, registration, and system-preferred ordering. Start with the Authentication methods policy and the user's usable methods.
  • The method exists but cannot be enabled: inspect provider application consent and the administrator role used to grant it.
  • The user never leaves Microsoft Entra: determine whether Conditional Access required MFA, whether the user was in method scope, and whether another method or claim already satisfied MFA.
  • The browser reaches the provider and fails there: correlate the provider account, policy, availability, and request-validation evidence with the Entra attempt.
  • The provider succeeds but Entra rejects the return: inspect OIDC metadata, signing keys, issuer, audience, nonce, subject, acr, and amr.
  • The user sees two provider prompts: inspect all applicable policies for custom-control and External MFA overlap.
  • Report-only looks healthy but the enabled policy fails: perform a new live sign-in and read the complete policy set; report-only did not execute the external challenge.
  • Windows 10 setup cannot continue: treat this as an unsupported OOBE path and use a supported Windows 11 or other approved onboarding method.

Keep the words enabled, registered, and required separate. Enabled means the method policy permits the user to use the provider. Registered means Entra has a usable external-method registration for that user. Required means a sign-in policy actually asks for MFA. One state does not prove the other two.

How the External MFA control plane works

External MFA is not a custom redirect bolted onto a Conditional Access policy. The provider is registered as an authentication method in the tenant's Authentication methods policy. When a sign-in requires MFA, Microsoft Entra evaluates the tenant policies, offers an eligible external method, redirects the browser to the provider's OIDC authorization endpoint, and validates the provider's signed response before treating the MFA requirement as satisfied.

Microsoft's provider reference documents the control-plane sequence: the first factor completes with Entra, the external provider supplies a complementary second factor, and Entra validates both the provider signature and the returned claims. Entra can still deny access for another Conditional Access requirement after external MFA succeeds. The External MFA provider reference documents the redirect, token, and validation sequence.

The integration depends on three public provider surfaces:

  • an HTTPS OIDC discovery endpoint ending in /.well-known/openid-configuration;
  • the authorization endpoint published by that discovery document; and
  • a public JSON Web Key Set endpoint used to validate provider signatures.

That architecture gives you a useful boundary. If the browser never redirects, investigate Entra scope and policy. If it redirects and the provider never accepts the request, investigate the provider and the request it received. If the provider completes the factor but Entra rejects the response, investigate the return token and the metadata Entra uses to validate it.

Prerequisites that must all be true

Microsoft's current migration guidance requires Microsoft Entra ID P1 or P2, an Authentication Policy Administrator or Global Administrator to configure the method, a Privileged Role Administrator to grant admin consent to the provider application, provider-supplied Application ID, Client ID, and OIDC discovery URL, and a controlled test group. The migration guide lists the licensing, roles, metadata, and pilot prerequisites.

Before troubleshooting a user, record:

  • the external method's display name and enabled state;
  • included and excluded groups;
  • provider Application ID, Client ID, discovery URL, and consent state;
  • the user's object ID, tenant, group memberships, and external-method registration;
  • every Conditional Access policy that applies to the test sign-in;
  • target application, client type, device platform, location, and exact UTC time;
  • the user-visible error, correlation ID, request ID, and provider event ID when available.

Do not grant Global Administrator simply because the integration needs consent. Separate method configuration from application consent when your change process allows it, and return any temporary privileged access after the approved task.

In Entra ID > Authentication methods, open the external method and inspect its consent and enablement state. Microsoft permits the configuration to be saved while consent is missing, but the method cannot be enabled successfully until the provider application has consent. If the application is later deleted or loses the required permission, users receive an error and cannot use the method. Microsoft's management guide documents both failure states.

If the Entra error is AADSTS900491 and says the service principal for the provider App ID was not found, Microsoft's provider reference identifies missing tenant admin consent as the cause. Verify that the Application ID is the one supplied for your cloud and tenant scenario; do not create a look-alike enterprise application to make the message disappear.

Also confirm that the configured discovery endpoint and Client ID belong to the same provider configuration. The display name is user-facing and cannot be renamed after creation, but a cosmetic name problem does not explain a failed token validation.

Step 2: prove method scope and user registration

Open the external method under Entra ID > Authentication methods and compare its include and exclude targets with the affected user. Expand nested group membership where your design relies on groups, and check that the user is in the tenant where the method is configured.

Then open Users > All users > [user] > Authentication Methods. A scoped user can register the method through Security info or the registration wizard, and an administrator can add or delete an external-method registration for recovery. Microsoft's management guide documents those supported registration paths and notes that deleting a user's registration causes a new registration flow at the next sign-in.

Be careful with reports. The same Microsoft page says users enabled through groups are not included in authentication method registration reports. An empty population report therefore does not prove that group-scoped users cannot use External MFA. Use the user's individual Authentication Methods view and a controlled sign-in as the decisive evidence.

If the method is not the default choice, check Use a different verification option before calling it missing. With system-preferred authentication enabled, Entra can present another registered method first. External MFA remains selectable when the user is enabled for it.

Step 3: check the Conditional Access grant

The supported grant is Require multifactor authentication. Do not use Require authentication strength for this integration: Microsoft says External MFA is not currently compatible with authentication strength policies. The migration guide states this limitation beside the test-policy design.

For a safe diagnostic policy:

  1. Target a small test group and the same applications and conditions as the intended production policy.
  2. Exclude the tenant's documented emergency access accounts.
  3. Select Grant access > Require multifactor authentication.
  4. Begin in report-only mode to inspect policy targeting.
  5. Turn on the policy only for the test group when the evaluation is correct.

Report-only is a scope check, not proof that the provider can complete a live factor. After enablement, generate a fresh test sign-in and inspect its actual result.

Read every policy on the sign-in, not only the one with External MFA in its change ticket. The site's Conditional Access evaluation pipeline explains why multiple applicable policies combine rather than replace one another, while the MFA rollout strategy provides the ring and emergency-account design around the change.

Step 4: stop double prompts from overlapping controls

External MFA and legacy custom controls can operate in parallel, but the same user must not be targeted by both during migration. If both policies apply, Microsoft says the user must satisfy the External MFA requirement and the custom control, causing a second redirect to the provider. Microsoft documents the parallel-policy behavior and recommends distinct test populations.

For each migration ring:

  1. include the ring in the new policy that requires MFA;
  2. exclude that same ring from the old custom-control policy;
  3. use Conditional Access What If to confirm the intended policy set;
  4. perform a live sign-in and verify one external challenge;
  5. expand only after the logs and user experience agree.

Do not delete the old policy during the pilot. Disable it only after all intended users are migrated and keep the approved configuration available for rollback while the External MFA path proves stable.

Step 5: diagnose the OIDC handoff

When the browser reaches the provider, ask the provider team to locate the exact request by time and client-request-id when available. Confirm that the provider validated the id_token_hint from Entra and returned the response to the correct cloud-specific redirect URI.

For a failure after the provider reports success, validate these documented requirements:

  • the discovery URL uses HTTPS and ends exactly with /.well-known/openid-configuration, without a query string or fragment;
  • the issuer matches character-for-character across the configured value, discovery document, and returned iss claim;
  • the authorization_endpoint is registered as an allowed reply URL on the provider application;
  • jwks_uri is HTTPS and its keys include the required x5c certificate chain value;
  • the response is signed with RS256 and the signing key can be selected by kid;
  • the returned audience, subject, nonce, state, acr, and single amr value match the request and Microsoft's External MFA contract.

Microsoft also warns that the kid value must use consistent base64 encoding in the provider token header and JWKS. A mismatch can look like a healthy provider challenge followed by an unexplained Entra rejection. The management guide calls out this exact signature-validation dependency.

Key rollover deserves its own incident branch. Microsoft Entra caches provider metadata, including signing keys. The provider reference says the cache refreshes every 24 hours and recommends overlapping old and new certificates during rollover. If failures begin at a certificate change, compare the signing kid with both keys currently published by the provider instead of repeatedly editing the Entra method.

Step 6: read the sign-in evidence in order

Go to Entra ID > Monitoring & health > Sign-in logs and locate the attempt by user, application, time, correlation ID, and request ID. Then read:

  1. Basic info for the final sign-in result and resource.
  2. Authentication Details for the sequence, method, success or failure, and whether MFA was satisfied by an existing claim.
  3. Conditional Access for every evaluated policy and grant result.
  4. Device, Location, and client details for conditions that can deny access after MFA succeeds.

Microsoft's MFA reporting guide confirms that Authentication Details shows the method sequence and failure reason, while the Conditional Access tab identifies the policy that triggered the MFA requirement. It also warns that newly written authentication details can initially be incomplete until aggregation finishes. Use the Microsoft sign-in reporting fields and respect that aggregation caveat.

The site's Conditional Access sign-in-log field guide is the companion procedure for interpreting those tabs. Preserve the original record before retesting; a later success does not erase the evidence needed to explain the failure.

Failure patterns and the narrow fix

External MFA is absent. Confirm method targeting, exclusions, individual registration, and the alternate-method picker. Do not treat a registration report as authoritative for group-scoped users.

The method is disabled after saving. Obtain Privileged Role Administrator consent for the correct provider application, allow the consent change to replicate, and then enable the method. Do not substitute a new App ID.

Another method appears first. This can be system-preferred authentication, not a failure. Select another verification option and confirm whether External MFA is available.

The user is never redirected. Confirm that an enabled Conditional Access policy actually requires MFA for this user, resource, and sign-in context. Check whether an existing MFA claim already satisfied the grant.

The user is redirected twice. Remove the pilot group from either the custom-control policy or the External MFA policy so it is not subject to both.

The provider succeeds and Entra fails. Preserve both event records. Check consent, issuer equality, audience, subject, nonce, acr, amr, signing algorithm, kid, x5c, and current JWKS publication.

Failures begin during provider certificate rotation. Verify that old and new keys overlap long enough for Entra's cached metadata to refresh. Escalate with the first failing time and both key identifiers.

External MFA works in web apps but Windows 10 OOBE stops. Microsoft says Windows 10 OOBE does not natively support External MFA and recommends Windows 11. Do not weaken the tenant-wide MFA policy to repair an unsupported device-setup path. The platform limitation is documented in the External MFA management FAQ.

Safe rollout, rollback, and escalation

Use a ring-based rollout with one explicit exit condition per ring:

  • Lab: provider metadata, consent, one test user, one low-risk app, and one controlled MFA policy.
  • Pilot: representative devices, browsers, locations, guest/member types where applicable, provider recovery, and sign-in frequency behavior.
  • Early adopters: production applications and help-desk monitoring, with the old custom control excluding only the migrated population.
  • Broad rollout: expand method and policy scope together, then confirm success rates and failure reasons before the next ring.

Rollback the smallest changed layer. If the new Conditional Access policy caused the incident, disable that pilot policy and restore the pilot group's previous policy assignment. If provider consent or metadata is broken, remove the affected users from the External MFA pilot scope while preserving another approved MFA path. Keep emergency access accounts governed by their existing documented procedure.

Do not delete the External MFA configuration, custom-control policy, provider enterprise application, or user registrations during first response. Those actions remove evidence and make a controlled rollback harder.

Escalate to the provider when Entra generated the redirect and the provider rejected or failed the request. Escalate to Microsoft when the provider can prove it returned a contract-compliant signed response and Entra rejected it, or when Entra's sign-in and policy records contradict the configured state. Include tenant ID, user object ID, UTC time window, application, correlation and request IDs, policy IDs, provider event ID, discovery URL, issuer, signing kid, and sanitized token-validation error. Do not send raw tokens or secrets in a support ticket.

Administrator checklist

  • Confirm External MFA is GA and the affected scenario is in its documented scope.
  • Verify Microsoft Entra ID P1 or P2 licensing.
  • Confirm the provider Application ID, Client ID, discovery URL, and admin consent.
  • Confirm the external method is enabled for the affected user and not excluded.
  • Verify the user's individual External MFA registration.
  • Use Require multifactor authentication, not authentication strength.
  • Exclude emergency access accounts under the tenant's approved design.
  • Keep pilot users out of the legacy custom-control policy to prevent double prompts.
  • Test report-only targeting, then perform a live enabled-policy sign-in.
  • Correlate Authentication Details, Conditional Access, and provider evidence.
  • Validate issuer, audience, nonce, subject, acr, amr, kid, x5c, and key rollover when the return fails.
  • Keep the old configuration disabled rather than deleted until rollback is no longer required.
  • Escalate with identifiers and sanitized evidence, never raw tokens.

The operational rule is straightforward: Entra decides whether MFA is required, the external provider performs the second factor, and Entra decides whether the returned proof satisfies the request. Find the first boundary without evidence of success and repair that boundary—without dismantling the controls around it.
